I understand that per my contract, I am required to give 90 days’ notice so that you can secure another credentialed provider. Unfortunately, that’s not possible, and thus I understand that I forfeit my Paid Time Off.
Given the severity and immediacy of my personal situation, I would greatly appreciate it if admin and HR would review my record of dedicated service during the contract period and consider paying out my accrued PTO.
