If it's possible, I would like to apply for a leave of absence of three months, beginning June 1st.

My wife is enduring a difficult pregnancy/ my husband has to take a course of chemotherapy/ my father is on his deathbed and I have to be there to look after him/her as best I can.

If this can't be arranged then I must regretfully submit my resignation, effective Friday, June 1. I do not wish to go to such drastic lengths but the situation is such that I can't worry about a job when my (husband etc.) is in such desperate difficulty.
